How we can work together
Curated Medicine is a membership-based family medicine practice built around you. No insurance billing, no crowded waiting rooms — just direct access to your physician through telehealth, with two membership options designed to fit your life. Both memberships include between-visit messaging through your secure patient portal and priority scheduling for acute concerns.
Services are available to patients physically located in Missouri.
Signature Membership
$250/month — No per-visit charges
The Signature Membership is designed for patients who want direct, ongoing access to their physician with a straightforward, predictable monthly fee. Your membership covers all scheduled primary care visits with no additional charges per visit.
What's included:
All scheduled telehealth visits for acute, chronic, and preventive care
Between-visit messaging through your secure Cerbo patient portal
Care coordination, specialist referral coordination, and prescription management
Review of outside records, lab results, and test results
Enrollment fee: $250 (non-refundable)
Essential Membership
$150/month — Per-visit fees apply
The Essential Membership provides access to scheduled primary care visits and between-visit messaging through your secure patient portal. Visits are charged separately, and a superbill is available after each visit for submission to your insurance carrier as an out-of-network claim.
What's included:
Scheduled telehealth visits for acute, chronic, and preventive care
Between-visit messaging through your secure Cerbo patient portal
Care coordination, specialist referral coordination, and prescription management
Superbill provided after each visit for potential insurance reimbursement
Visit fees: Initial Visit (90 min) $350 | Follow-Up (55 min) $250 | Follow-Up (25 min) $150
Note: Medicare patients may not submit superbills to Medicare for services provided by Curated Medicine.
Your First Visit
GETTING STARTED
Your first visit is scheduled for up to 90 minutes and takes place via telehealth through a secure video link sent to you through your patient portal. Before your visit, you will complete intake forms through the portal, including your medical, family, and surgical history, current medications and supplements, and any relevant documents you would like to share, such as prior lab results or health records.
This visit is time set aside to get to know you, your history, your current concerns, and your health goals.
